The patient had undergone double eyelid surgery 10 years ago, but over time, the crease became faint as the line loosened, and the eyes appeared weak and tired, leading to a desire for revision. Overall, both the horizontal and vertical dimensions of the eyes were short, and the inner and outer corners were not well-opened, giving the eyes a heavy and narrow appearance.
In this revision surgery using the incisional method, we combined ptosis correction, medial epicanthoplasty, and lateral plus lower canthoplasty to maximize eye size, designing the results around the glamorous in-out crease the patient desired. Through ptosis correction, we enhanced the upward exposure of the eyes, opened the inner corners with medial epicanthoplasty, and lowered the outer corners diagonally with lateral and lower canthoplasty to create a wider, brighter eye shape.
As this was a revision case, we carefully released the existing adhesions, and focused on achieving a clear and symmetrical crease, taking into account the skin's elasticity and thickness. After surgery, the double eyelid line settled clearly, creating the defined and glamorous in-out line the patient envisioned, and the eyes now appear longer, sharper, and more three-dimensional from the front, resulting in a sophisticated and refined look.
